Hi
I am presently receiving a disability pension from a large company in Toronto.
It is so large that the right hand does not tell the left hand what is happening.
Last week I received a letter and questionnaire to fill out for this company and
just about everything asked is already in my file.
The part that really upsets me is: My Name and Address was typed on top:
The salutation was addressed to someone else AND it had this persons ID# and File#noted.

To me this is a grave error on the Company;s part, and leaves me wondering Did this other person receive my personal information?

I would like to do something about this but I am not sure what to do.
Any suggestions would be appreciated.
