Where can I get a Witness statement Template?

I wish to provide a witness statement for a criminal case in Ca.

Is it possible to obtain an MS Word template, for completing such a statement? What form number is it?

You assume that I am an Atorney? I am not
I am the witness, and I know how to prepare a statement.

I am am from the UK, and do not know what the headers are on a US statement form.

Here is an example of a UK Header.

Witness Statement
(CJ Act 1967, S.9; MC Act 1980, SS.5A(3) (a) and 5B; Criminal Procedure Rules 2005, Rule 27.1)

This statement (consisting of page(s) each signed by me) is true to the best of my knowledge and belief and I make it knowing that, if it is tendered in evidence, I shall be liable to prosecution if I have wilfully stated in it, anything which I know to be false, or do not believe to be true.

Now what I need to know is does a US statement have a similar header
which it needs, to be admissible in a US criminal court?
